We start with a conversation, develop the care plan around your loved one’s routine, and keep the same caregivers coming back.

Caregiver matching

Clients are matched with caregivers based on needs, personality and preferences, not just availability. A small, consistent team (usually two caregivers, who rotate) provides the care, so your loved one develops comfortable and confident connections.

An RN-led care plan

A registered nurse writes the plan, trains the caregivers on it and updates it as needs change, such as after a fall, hospital stay or new diagnosis.

Hours that fit real life

We provide services to fit your loved one’s schedule and needs, whether that’s four hours twice a week, overnights, weekends, or live-in care. There is no long-term contract, so it’s easy to change the schedule as circumstances change.

Home care works best when it’s built around the person receiving it. We get to know your loved one’s needs, routines and preferences, then create a care plan designed specifically for them. Care can often begin within 24 hours of your first call. Here’s how it works:

We have a real conversation, free of charge
We sit down with you and your loved one, in the home when possible, and talk through the day: routines, meals, medication, mobility, where help is needed and what’s important to keep the same.

Your RN case manager develops the care plan
A licensed registered nurse turns our conversation into a personalized care plan, including specific needs, safety considerations and preferences. Each assigned caregiver is prepared to follow the plan before care begins.

We find the right caregiver match
Our meticulous caregiver matching process considers more than just care needs and shift availability. Personality, language, pets, interests, routines and other preferences can all help create a more comfortable connection. And if the fit doesn’t feel right, we’ll work with you to find a better one.

We adjust as needs change
Care needs evolve and the care plan will evolve with them. Through supervisory visits, family check-ins and ongoing RN oversight, we make adjustments so the support continues to fit your loved one.

About Baywood

Started by a Minnesotan who thought care could be warmer

Baywood Home Care was founded in 2000 by Dr. Dorothy Muffet, who traded a successful corporate career to create a home care model centered on people, not systems. Her mission was to deliver exceptional care in every setting and empower individuals to live life fully, on their own terms.

Since Dorothy’s passing in 2022, her legacy has been carried forward by Henrietta Nayoh, one of Baywood’s first employees. Henrietta started as a caregiver and moved into successive roles of increasing responsibility, becoming Dorothy’s second in command. Today, she leads the team of nurses, office staff and caregivers who collectively bring our rich history, decades of service and years of experience to serve families in the Twin Cities and Greater Minnesota. Our services are designed to offer freedom and eliminate limitations for our clients to remain in their homes, making Baywood the top choice for personalized in-home care in Minnesota.
